Item details
A fillable PDF permission slip: click a field and type, then save or print. Parent or guardian consent for a school activity.
Fields and sections on the page: student name; grade; date; activity or event; date of activity; location; cost; tick boxes (i give permission for my child to take part; i do not give permission; i can volunteer as a helper...); medical or dietary notes.
Colours follow a Brick palette with main colour #9A3B2F and accent #2F4F4F. Layout is a single page; the 16 fields are input areas arranged to match the labelled rows and sections. Headings are set in Lora (SIL Open Font License, embedded in the PDF), body labels in Nunito.
One ZIP holds the whole product: a US Letter PDF, an A4 PDF and a short read-me with editing tips.
To fill in: open the PDF in a reader, click a field and type. Field contents can be saved in most readers; if yours cannot save form data, print the page to PDF after typing. The page is not an editable layout file; use it as a ready-made form.
Works with PDF readers that support fillable text fields and checkboxes. For personal and classroom use, or in your own small business paperwork. Files are digital downloads; nothing is shipped.
