Page one is a full-page letter home with trip details (destination, date, bus times, cost, lunch, return date), a chaperone note and a complete permission section with emergency contact, allergy and medical box, check options and signature line. Page two repeats the essentials as a short form, two slips per sheet, with a cut line so you can copy and send them home fast.

Compatibility: - DOCX opens and edits in Microsoft Word (2016, 2019, 2021, Microsoft 365), Google Docs (upload to Drive and open with Docs), LibreOffice Writer and Apple Pages. - ODT opens and edits in LibreOffice Writer, Apache OpenOffice, Microsoft Word, Google Docs and Apple Pages.
Fonts used: Fraunces, Nunito. They are free from Google Fonts under the SIL Open Font License, including commercial use. Install them before opening for the intended look; without them, a close fallback font is substituted automatically and spacing may differ a little.
Every blank is a fill-in line or box: type the details into the document before printing, or print as is and write them in by hand. Check the final text against your own school or district policy; this template is a layout, not legal advice.
